SMPHOG Membership and Release Form:     

This is the form needed to join our chapter.  It is also a release form.    

Membership dues $15.

Adult EventRelease Form:            

All SMPHOG non-members must sign this release to participate in any rides or activities.

Minor Event Release Form:

This form is used if any minors participate in any rides or activities.  

It is completed and signed by the parents or guardian.

Minor Assumption of Risk Acknowledgment:

This form is signed by minors who are 12 or above.    

Both Minor Assumption of Risk Acknowledgment and the Minor Event Release Form

                 must be submitted together.
